2010 Summer Volunteer Evaluation
The intention of this evaluation is to pinpoint both what went well and not so well. If you have something critical to say, we want to hear it so we can improve what we are doing. We thank you for your time and honesty.
Please answer the questions with reference to your 2010 summer volunteer experience, not to any previous experiences.
This form should take about 15–20 minutes to complete. PLEASE FILL IT OUT COMPLETELY. If any required information is missing, you will be prompted for it. For an irrelevant technical reason, YOU WILL ALSO HAVE TO RE-ANSWER THE MULTIPLE-CHOICE QUESTIONS THAT INVOLVE CHECKBOXES.
If you have any technical difficulties with this form, please contact the Volunteer Coordinator at This e-mail address is being protected from spambots. You need JavaScript enabled to view it . You should see a big "THANK YOU" confirmation screen at the end indicating that your answers were successfully recorded.
